Health Insurance Information

We take many  insurance plans as well as self-pay patients.

You should check with your insurance plan at least 2 days before your appointment to check on eligibility, referrals, co-pays, deductibles, benefits, etc. 

Do not assume that because your last office visit was covered by your insurance that you still have the same benefits or even insurance.

Always bring your insurance card with you.

We will submit primary and secondary insurance claims on your behalf.

If your insurance company is not listed, we suggest you call the office at 954-467-4100 and ask if we accept your insurance.
